I understand that HACK Performance Labs does not provide medical advice, medical diagnosis, or the practice of medicine. Any communications, recommendations, or protocol suggestions are educational and research-oriented in nature. I agree that I am solely responsible for consulting with my own licensed physician or qualified healthcare provider before beginning any protocol.

By signing this waiver, I waive any right to claim that HACK Performance Labs owes me a duty of medical care, medical advice, or ongoing physician supervision.

NO LAB CHECK OBLIGATION

I understand that HACK Performance Labs is not responsible for ordering, reviewing, interpreting, or monitoring laboratory tests, blood work, biomarkers, or any other diagnostic evaluations. I acknowledge that any lab testing I choose to obtain is my own responsibility, and I waive any right to demand that HACK Performance Labs perform, review, or act on lab checks on my behalf.

ASSUMPTION OF RISK

I am aware that research compounds and peptides may cause adverse reactions, side effects, allergic responses, unexpected health effects, or interactions with medications or supplements I am currently taking. I voluntarily assume all risks associated with the use of these protocols, including but not limited to physical reactions, injury, illness, or other harms.

I release, waive, discharge, and covenant not to sue HACK Performance Labs, its owners, officers, employees, contractors, physicians, affiliates, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any adverse reactions, injuries, or other effects I may experience.

VOLUNTARY PARTICIPATION

I certify that I am at least 18 years of age and that I am participating voluntarily. I have read this waiver carefully, understand its contents, and agree to be bound by it. I understand that I may stop participating at any time, but that this waiver remains in effect for any protocols already initiated.
